Step 1: Complete the New Client Request Form below.

Step 2: Within 24-48 business hours our intake team will then send you the full intake paperwork to digitally fill out in the secure client portal. Complete all the required intake forms & upload clear copies of the front & back of all insurance card(s). *Please note, that the system will not show you as ready to schedule until ALL forms have been submitted.

Step 3: Once this is submitted we will review to be sure we’re a good clinical fit, verify your insurance, and assign you a clinician to get you scheduled with.
